Complete this form when you get a NEW JOB, or when you CHANGE JOBS.




The Follow-Up phase of the program now begins for the next six months.  You must hold the job for 14 days before you are considered to be “placed”.  Key dates for contact with your counselor are:  when you get job confirmation, 14 days, 90 days, and 180 days after you get a job and anytime in between.  In the Follow-Up phase, your counselor will help you with anything that comes up on the job including on-site visits if you want.  In-office visits on your days off can also be arranged.  It’s important that contact with the counselor be maintained.  Once you get a job, it’s important too that you be working at 14 days, 90 days, and 180 days.  It does not have to be in the same job, it can be in a different job, if the first one doesn’t work out for whatever reason.  If you must leave your first job (quit, laid off, dismissed), assistance will be provided to find subsequent jobs during this Follow-Up phase.  If you have any questions, don’t hesitate to contact your counselor.
